UnitedHealthcare is a broad national brand used across multiple insurance products, legal entities, networks, and markets. A national website or logo does not establish that an individual Marketplace plan is offered in a particular county.
Match the state, product line, exact plan name, plan ID, and network. Employer, Medicare, Medicaid, short-term, and individual Marketplace products can have different documents and directories.

- State, county or ZIP code, and plan year
- Coverage market: Marketplace, employer, Medicaid, Medicare, or another product
- Legal issuer and full plan name
- Plan ID and named network
- Current SBC, policy, provider directory, and formulary
- Official availability result and date checked

Confirm after you shortlist
Save the directory result and call important provider offices with the full plan name and network. Search each regular prescription by drug, dose, and form. Ask the plan which document controls if the SBC, directory, formulary, or website appears inconsistent.
