A source-based process for correcting material facts, broken official routes, and ambiguous coverage language.
Report an issue
Email [email protected] with the page URL, the statement or link in question, the official source showing the conflict, and the date checked. Do not attach personal health, identity, financial, claim, or application records.
Review process
We check the controlling record, current official source, exact plan or program context, and publication date. If a material factual error is confirmed, the page should be corrected, its update date changed, and a note added here when the correction could affect a reader's decision.
