Evidence-based coverage guide
Individual health insurance
Find the official Marketplace, separate on- and off-Marketplace routes, and compare current plan documents.
Individual coverage is purchased for a person or family rather than provided through an employer group. Marketplace and off-Marketplace individual plans can use private insurers, but Marketplace savings are determined only through the official Marketplace.
Start with the state route and live address-specific results. Compare the exact plan ID, net premium, network, prescriptions, cost sharing, out-of-pocket protection, quality rating when shown, and enrollment dates. Identify limited-benefit or short-term products separately from individual major medical coverage.
Verification checklist
Collect the controlling facts
- Official state Marketplace route
- Marketplace versus off-Marketplace status
- Eligibility result and net premium
- Plan ID, SBC, policy, network, and formulary
- Enrollment and effective dates
- Issuer payment and activation confirmation
Reusable process
Turn information into a dated record
- Start at the responsible official source. Confirm the program, plan, Marketplace, or agency that controls the question.
- Match identity and date. Use the exact plan, coverage year, notice, person, service area, or program record.
- Write unknowns as unknown. Ask the responsible organization instead of filling a gap with a national average.
- Keep the documents. Save the current notice, policy, result, reference number, and the date checked.
When this topic needs another check
Review the official source again when the plan year changes, the household moves, eligibility facts change, a new notice arrives, a provider or prescription changes, or an important service is planned. Saved web pages and directories are dated evidence, not permanent promises. Keep new records beside the earlier version so a change is visible.
Coverage routes
Identify the individual plan market
Use the same evidence fields after the route is known.
| Route | Where to begin | Key distinction |
|---|---|---|
| Marketplace individual plan | Official federal or state Marketplace | Only the official route can determine Marketplace savings |
| Off-Marketplace comprehensive plan | Issuer or licensed channel | Confirm ACA status, protections, and exact documents |
| Public program | Official Medicaid, CHIP, or Medicare process | Eligibility and coverage follow program-specific rules |
| Temporary or limited product | Policy and state availability record | Do not treat limited benefits as equivalent to comprehensive coverage |
